New Hire or New to Benefits

When you enroll in coverage, you may be able to cover your eligible dependents, including your:

• Spouse
• Children (including stepchildren, legally adopted children and children for whom you are the legal guardian) through December 31 of the year they turn 26

You’ll need the following information for each dependent you want to enroll:

• Legal name
• Social Security number – If your dependent does not have a Social Security number, please call the HealthyME Benefits Center 866-959-3545 to enroll with a live customer service representative, Monday through Friday, 8:00 am to 8:00 pm, EST
• Date of birth

For certain coverages, you may be required to upload documentation to verify your relationship to each dependent (e.g., copy of a marriage certificate, birth certificate or adoption decree).

You have 30 days from your date of hire or eligibility to enroll in any of the following programs, if eligible:

After this 30-day period, you can only enroll in or make changes to your benefits during fall Annual Open Enrollment or when you experience a Qualifying Life Event, such as marriage or birth of a child.

If You Don’t Enroll

If you’re eligible for benefits and do not take action to waive or enroll in coverage within 30 days, Montefiore will automatically enroll you in:

• Employee-only medical coverage in your available EPO plan (Registered Nurses Health Plan for NYSNA employees)
• Employee-only dental coverage in the Preventive and Diagnostic Dental Plan (Registered Nurses Dental Plan for NYSNA employees)
• Employee-only vision coverage (NYSNA employees only)
• Basic Life and AD&D insurance equal to 1x your annual base salary (up to $250,000)
• Mandatory Basic Long Term Disability

You will not have coverage for any family members.

Within 30 days of your date of hire or eligibility, log in to the HealthyME Benefits Center to enroll. Select "Register" to set up your username and password, or log in or Single Sign-On if you are accessing the site from within the Montefiore network. The system will walk you through each decision you need to make and all the information you need to enter. Follow the steps below or download these Enrollment Instructions:

• Click “Start Here” to review your personal information and add any dependents you wish to cover
  • Review and confirm your personal information. If you need to make changes to your name, address or other personal information after enrolling, log in to Workday and select “Personal Information”
  • Enter dependent information, including each dependent’s legal name, Social Security number and date of birth. If your dependent does not have a Social Security number, please call the HealthyME Benefits Center 866-959-3545 to enroll with a live customer service representative, 8:00 am to 8:00 pm, Monday through Friday

• You may elect or waive coverage in each benefit area. If you wish to waive coverage, you must actively elect that option
• You’ll be asked to indicate whether you use tobacco. If you have used tobacco products and answer “Yes” to the tobacco use question(s), you will be assessed a higher tobacco user premium on your medical (if any) and Supplemental Life Insurance
• Need help choosing? Select “I’d Like Help Choosing Plans and answer a few simple questions to find the plans that best fit your unique needs. Once completed, review your Best Match Results, based on your answers.
  • During enrollment you’ll have the opportunity to compare your available benefit plans and explore coverage options. You can also preview paycheck contributions and estimated costs you may incur throughout the year before finalizing your selections.
• Some plans, such as Life and Accidental Death & Dismemberment (AD&D) Insurance, require that you designate a beneficiary, which you’ll be prompted to complete during the enrollment process. Make sure you have the legal name, Social Security number and birthday for whoever you want to name as a beneficiary.
• If you have questions about enrollment, coverage or where to find more information, ask Sofia, your virtual benefits assistant
• Confirm your elections and save your email confirmation for your records

Once you've completed your enrollment, you can return to the benefits selection process and make changes as long as you’re still within your 30-day enrollment window.

After you enroll, you may be asked to provide additional information, including:

• Evidence of Insurability (EOI)—also called proof of good health—for yourself or your dependents. Follow the prompts as you enroll to submit EOI
• Verification of your relationship to a dependent (e.g., copy of a marriage certificate, birth certificate or adoption decree). You can review action that is required in My Tasks from the homepage of the HealthyME Benefits Center and will receive a notice by email and text, if you have opted in to receive text messages

Enroll in your Montefiore 403(b) Retirement Plan and set your contribution amount through Fidelity Net Benefits. Visit NetBenefits.com, and create a username and password to get started.
