The medical plan options available to you vary based on your role. You’ll receive information about your options when you’re eligible to enroll as a new hire and during Annual Open Enrollment, held each fall. If you experience a qualifying life event (e.g., marriage or a new child), you may be eligible to make a mid-year change.

Comparing Your Options

When you’re choosing a plan, consider:

Total annual costs. This means your contributions, annual deductibles, copays, coinsurance and the out-of-pocket maximums.
When and how you incur expenses. Some plans require higher paycheck contributions and lower annual deductibles, while others may offer lower contributions and higher annual deductibles.
Flexibility to choose your providers See details under Choosing Your Providers below.
Tax-advantaged accounts. You have the opportunity to use pre-tax dollars to pay eligible out-of-pocket expenses. Visit Spending Accounts for additional information.
Supplemental insurance. You can enroll in additional coverage that will pay a lump-sum amount if you experience an eligible event (e.g., accident or critical diagnosis). These plans provide financial support after an eligible event. They do not provide medical coverage. Log in to view the coverage available.

Choosing Your Providers

Each time you need care, you choose a provider. The tier the provider participates in will define your out-of-pocket cost.

Montefiore Tier 1 Network	Anthem Network	Out-of-Network
You want to pay the lowest amount for trusted, world-class care from Montefiore providers and selected Amazon One locations.	You want to use providers outside the Tier 1 Network and still save money. Anthem has vetted these providers and negotiated rates for covered services.	You want to use a provider that does not participate in either the Tier 1 or Anthem Network. Typically, you’ll pay more for care received out-of-network. Note: The PPO is the only plan that offers out-of-network coverage.
