A Free Benefit to Manage and Improve Your Health
The Care Guidance Program provides the Montefiore Health System (MHS) employees, and their dependents covered by the MHS Anthem medical plan* with a free and valuable health benefit that can help you manage and improve your health.
Care Guidance provides you access to a Personal Health Nurse (PHN). Working as a team, you, your physician, and PHN will set health goals, create an action plan, and identify ways to help you maintain healthy habits. Your PHN will reach out by phone regularly to help guide you through the healthcare system, making your care more manageable and successful. Your PHN will work with you for as long and as often as you need. Once you've started working with a PHN, you can opt-out at any time.

How Care Guidance Can Help
Assist with chronic illness, symptom management, and achieving personal health goals
- Call following Hospital and Emergency Department discharge
- Help establish new care with providers in the Montefiore network
- Coordinate care with your providers
- Connect you with behavioral health and community resources
- Direct you to stress management, weight loss, fitness, and smoking cessation resources
- Provide a personal nutritional consultation with a Registered Dietitian for weight loss and diabetes management
- Offer pharmacist review of medications and guidance on medication management
- Furnish educational information on a wide range of health topics
- Assist with transition back to work following a medical leave.
